In the ‘Product Information’ section, specify each product provider and the corresponding policy/account number for all relevant products for which Edale requires authority. You can add multiple product entries by clicking the “Add Another Product” button.
Use your finger or mouse to provide your signature in the designated box, and then enter your name and the current date.
Once all fields are completed, click the “Generate & Download PDF” button to save a signed copy of this letter to your device.
